Where necessary for medical legal reasons, moving away or any other reason, our practice will transfer medical records it holds on behalf of your doctor upon signed receipt of a file transfer request.
All persons aged over 14 must sign the authority to release.
Upon receipt of the request for records, a confirmation letter will be sent advising of the necessary fee for file transfer.
The records will be transferred within 14 days of payment of this fee.
