Advance Care Planning
Advance care planning allows health professionals to understand and respect a person’s preferences, if the person ever becomes seriously ill and unable to communicate for themselves. People of all ages are encouraged to prepare their personal end of life care plans, to give certainty to themselves and their families.
GP Chronic Condition Management Plan (GPCCMP):
GPCCMP’s are a plan designed to make it easier for you and your doctor to manage chronic or long-term health conditions, such as diabetes, arthritis, heart disease, or asthma. The GPCCMP is a single, streamlined care plan that outlines your health goals, treatment options, and any support you may need, including referrals to allied health providers like physiotherapists or dietitians.
The plan is prepared by your regular GP and is tailored specifically to your health needs. It allows access to Medicare-subsidised services, including up to five allied health appointments per year. Your doctor will also review your plan with you regularly—ideally every three months—to make sure it stays relevant and effective.
